Requires the Department of State Health Services to make immunization exemption affidavit forms available in printable format on the department's website and in all school nurse offices, in addition to the existing written request process

Removes the requirement that affidavit forms contain a seal or security device to prevent reproduction

Prohibits requiring individuals to provide any personal information to obtain a blank affidavit form

Prohibits the department from maintaining records of any personally identifying information of individuals who download, access, request, or submit an affidavit form; tracking of total form accesses becomes optional rather than mandatory

Applies beginning with the 2025-2026 school year and takes effect immediately with two-thirds vote or September 1, 2025
