Establishes a pilot program for Medicaid reimbursement of doula services, defined as nonmedical childbirth education, coaching, and emotional/physical support during pregnancy, labor, delivery, and the postpartum period

Requires implementation by September 1, 2026 in two counties: the most populous county in Texas and the county with the greatest maternal health support needs based on maternal/infant mortality rates and Medicaid births

Directs the executive commissioner to establish doula qualifications and covered services in consultation with the Perinatal Advisory Council

Mandates annual reports evaluating program costs and impact on birth outcomes, with the first report due September 1, 2027, and a final legislative report due September 1, 2030 assessing effectiveness in reducing maternal mortality and health disparities

Pilot program terminates and the statutory section expires on September 1, 2031
