Amends the emergency detention notification form that peace officers must file when transporting a person evidencing mental illness to a mental health facility, requiring the form to be completed "in substantially" the prescribed format "to the best of the officer's ability"

Expands the required form content to include detailed demographic information (DOB, race, gender, address, phone number), whether the person is a minor or elderly adult, parental notification status for minors, use of restraints, call origin location, and firearm possession/seizure information

Adds new observation/history checklist sections for officers to document harm to self/others, previous suicide attempts, psychiatric treatment history, prescribed medications, sleeping difficulty, and substance use disorders

Removes the previous requirement for a separate "specific description of the risk of harm" while retaining the requirement to document specific behaviors, acts, attempts, or threats supporting the officer's beliefs

Requires mental health facilities and hospital emergency departments to provide peace officers with a copy of the completed notification form upon request, and mandates that the officer's employing law enforcement agency retain a copy; effective September 1, 2025
