Requires the Texas Department of State Health Services to post a printable immunization exemption affidavit form on its website, rather than requiring individuals to submit a written request as the only method to obtain the form

Removes the requirement that the affidavit form contain a seal or security device to prevent reproduction

Prohibits requiring individuals to provide any personal information to obtain a blank exemption affidavit form

Prohibits the department from maintaining records of personally identifying information for individuals who download, access, request, or submit an affidavit form

Changes record-keeping from mandatory annual reporting to the legislature to optional tracking of form access/requests

Applies beginning with the 2025-2026 school year; Department must modify the existing form within 30 days of the effective date
