Establishes a 14-member Mental Health, Substance Use, and Public Health Initiative Council to oversee fair allocation of funds and improve coordination of mental health, substance use, and public health services across Texas

Creates a trust fund held outside the state treasury by the Texas Treasury Safekeeping Trust Company, funded through legislative appropriations, earnings, and gifts/grants/donations

Council members appointed by the governor, lieutenant governor, speaker of the house, and executive commissioner must include healthcare professionals with 10+ years experience, representatives from rural hospitals, local health authorities, federally qualified health centers, and individuals with lived experience of mental health or substance use conditions

Grant program will fund evidence-based programs for individuals with co-occurring mental health and substance use disorders, supporting research, technology implementation, data collection, education, capital improvements, and provider sustainability

Requires unanimous council approval for grant awards, annual reporting to the legislature by October 1 on expenditures and fund balances, and biennial grant application evaluations in coordination with the statewide behavioral health coordinating council
