Amends the marriage license application form to replace "woman's maiden surname" with gender-neutral language requiring the surname of "an applicant intending to change the applicant's surname as a result of the marriage"

Modifies the informal marriage (common law) declaration form to use the same gender-neutral surname language for parties intending to change their surname

Changes the informal marriage declaration oath to replace "husband and wife" with "spouses" when describing how the couple lived together

Applies to both Section 2.004 (marriage license applications) and Section 2.402 (informal marriage declarations) of the Texas Family Code

Takes effect September 1, 2025
