Prohibits health benefit plan issuers from requiring preauthorization for screening mammograms and diagnostic breast imaging

Eliminates preauthorization requirements for breast reconstruction, surgery, prostheses, and related treatments following mastectomy, as well as associated inpatient care

Removes preauthorization requirements for diabetes equipment, supplies, and self-management training for qualified enrollees

Prohibits preauthorization for bone mass measurements to detect osteoporosis risk and for prostate cancer screening examinations for enrolled males

Applies to health benefit plans delivered, issued, or renewed on or after January 1, 2026, with the act taking effect September 1, 2025
