Requires the Health and Human Services Commission to develop a "friends and family form" allowing individuals with close personal interest in a patient to provide health and background information to assist with mental health, behavioral health, or substance use treatment

Friends and family may submit information including diagnosis history, past hospitalizations, de-escalation techniques, potential triggers, housing status, current medications, and family/social environment details, which may become part of the patient's medical record

Health care professionals discharging patients from inpatient mental health or substance use treatment must notify the patient's other treating providers of scheduled discharge and notify follow-up care providers within 24 hours of discharge with a medical history summary

Provides liability protection for health care professionals acting on information received through the friends and family form, and allows professionals to omit information if the patient objects to receiving it from a specific person or if deemed irrelevant

Commission must develop the forms, educational documents on self-harm warning signs, and implement rules by December 1, 2025, with the law applying to health care services provided on or after that date
