Defines "group home facility" as an establishment where three or more unrelated persons reside and receive paid residential care services including shelter, protection, meals, health care, mobility assistance, or personal care

Requires the Health and Human Services Commission executive commissioner to develop and publish model standards for group home facilities covering construction, sanitation, incident reporting, medication assistance, staff training, criminal background checks, and resident assessment

Authorizes counties and municipalities to require permits for group home facility operation within their jurisdiction, set reasonable fees, conduct inspections, and impose fines for noncompliance

Creates a Class B misdemeanor for operating a group home facility without required local permit, and a Class A misdemeanor for building owners who fail to report known abuse, neglect, or exploitation of facility residents

Mandates reporting of suspected abuse, neglect, or exploitation to the Department of Family and Protective Services, prohibits retaliation against employees who report, and requires annual reporting by counties/municipalities to HHSC with biennial legislative reports
