Forms must document the date, time, type, and location of each client interaction, along with the name of any employee who provided services

Requires an itemized list of each service, material item, or referral provided to each unduplicated client during every interaction

Health and Human Services Commission must develop and publish the required form by November 1, 2025, with providers required to use the form starting December 1, 2025
