Reduces network adequacy coverage requirements from 100% to 75% of insureds, meaning preferred provider benefit plans must only ensure access to in-network physicians and healthcare services for at least 75% of plan members rather than all members

Increases maximum travel time and distance requirements across all county types (large metro, metro, micro, rural, and extreme access counties), with increases ranging from 5-30 additional minutes and 5-30 additional miles depending on provider type and location

Removes appointment availability/wait time standards from network adequacy requirements, eliminating previously mandated maximum wait times for scheduling appointments with preferred providers

Eliminates public hearing requirements for network adequacy waiver requests and removes limits on the number of consecutive or total waivers an insurer can receive for the same network adequacy standard

Repeals Section 1301.00555 (appointment wait time standards) and Section 1301.00565 (public hearing requirements), with changes applying to policies delivered, issued, or renewed on or after January 1, 2026
