Authorizes eligible counties to establish a health care provider participation program that collects mandatory payments from nonpublic hospitals providing inpatient services, with payments deposited into a local provider participation fund

Applies only to counties not served by a hospital district that either: (1) have 1 million+ population, contain part of a municipality with 1 million+ population, and are adjacent to a county with 2.5 million+ population, or (2) have 900,000+ population and border two counties each with 2 million+ population

Caps aggregate mandatory payments at 6% of total net patient revenue from hospital services provided by all paying providers in the county, with payments based proportionally on each provider's net patient revenue

Limits fund usage to intergovernmental transfers for Medicaid supplemental payments (uncompensated care, rate enhancements, directed payment programs), county administrative expenses up to $150,000 annually, and refunds to providers

Prohibits using funds to expand Medicaid eligibility under the Affordable Care Act or to fund Medicaid disproportionate share hospital payments, and the program expires December 31, 2030
