Authorizes certain Texas counties not served by a hospital district to establish health care provider participation programs that collect mandatory payments from nonpublic hospitals providing inpatient services

Applies to three specific county types: counties with population 46,000-50,000 adjacent to the state capital county; counties with 900,000+ population bordering two counties each with 2+ million people; and counties with 1+ million population containing a municipality of 1+ million and adjacent to a county of 2.5+ million

Mandatory payments are assessed quarterly based on net patient revenue, with aggregate payments capped at 6% of total net patient revenue from all paying hospitals in the county

Funds collected must be deposited in a local provider participation fund and used primarily for intergovernmental transfers to provide the nonfederal share of Medicaid payments, including uncompensated care payments and rate enhancements for nonpublic hospitals

Administrative expense caps for counties range from $20,000 to $300,000 annually depending on county size, and hospitals are prohibited from passing mandatory payments to patients as surcharges
