Removes the requirement that a person must be "simultaneously" assisting "seven or more" voters to trigger documentation requirements when providing transportation to polling places for voters with disabilities

Requires any person who assists voters with disabilities by providing transportation to the polling place to complete and sign a form provided by an election officer

Expands required form information to include the voter's name and address, in addition to the existing requirement for the assisting person's name and address

Form must indicate whether assistance is being provided solely for transportation (curbside voting) or for both transportation and direct voting assistance under Subchapter B

Takes effect September 1, 2025
