Amends Section 36.002 of the Human Resources Code to expand requirements for health care program claims, requiring providers to indicate both the type of license held by the provider who actually provided the service and their identification number

Makes it an unlawful act to knowingly fail to disclose the license type of the health care provider who rendered services when submitting claims under a health care program

Maintains existing prohibitions against false statements, concealment of information, improper benefit conversion, fraudulent facility certifications, unauthorized payments, and claims for unlicensed or substandard services

Applies only to unlawful acts committed on or after September 1, 2025

Allows state agencies to delay implementation if federal waiver or authorization is required
