Requires health maintenance organizations (HMOs) to display the acronym "HMO" on identification cards issued to enrollees, with placement at the organization's discretion

Requires preferred provider organizations (PPOs) to display the acronym "PPO" on identification cards issued to insured individuals, with placement at the insurer's discretion

Establishes that the "HMO" or "PPO" designation on cards indicates that coverage through in-network providers does not guarantee access to discounted rates or fee discounts when using out-of-network providers

Applies only to identification cards issued on or after January 1, 2026

Takes effect September 1, 2025
