Requires the Texas Education Agency to create and maintain a comprehensive document informing parents of their rights regarding their child's health, including rights to withhold consent for health activities, access counseling and psychological records, review health instructional materials, exempt children from immunizations for medical or conscience reasons, and decline psychiatric examinations or psychotropic medications

Mandates school districts and open-enrollment charter schools to distribute the parental rights document before each school year via website posting, email to parents, enrollment materials, and when practicable in the parent's native language (must be available in English and Spanish)

Requires schools to notify parents of all health-related services offered on campus (including mental health counseling, screenings, and vaccination clinics) and provide at least 20 days advance notice before any health screening or clinic is conducted

Directs the agency to develop mandatory training on parental rights for school administrators, nurses, counselors, teachers, and relevant staff, and to establish a complaint procedure to investigate violations

Authorizes the commissioner to withhold state funding, assess administrative penalties, and conduct audits and site visits to enforce compliance; schools must publish annual reports on the number of complaints received; applies beginning with the 2025-2026 school year
