Establishes a minimum Medicaid reimbursement base rate for ground ambulance services at 40% of the Medicare rate for ground ambulance services originating in rural areas

Requires Medicaid managed care organizations to reimburse in-network ground ambulance providers at a rate equal to or greater than the Medicare rural rate

Mandates an automatic 3% annual increase to the ground ambulance reimbursement rate paid by managed care organizations

Applies to managed care contracts entered into or renewed on or after September 1, 2025; existing contracts may be amended to comply but original contract terms prevail if there is a conflict

Implementation may be delayed if federal waiver or authorization is required
