Requires elementary and secondary schools to provisionally admit students who have not obtained required immunizations or submitted an exemption affidavit, provided they show documentation that an affidavit request has been submitted to the Department of State Health Services

Mandates the Department of State Health Services to allow electronic submission of immunization exemption affidavit requests through the department's website

Requires the department to provide a standardized receipt confirming submission of the affidavit request, regardless of submission method (electronic, in person, mail, or fax)

Directs the Department of State Health Services to adopt rules governing provisional admission of students under this process

Applies beginning with the 2025-2026 school year and takes effect immediately with two-thirds legislative vote, or September 1, 2025 otherwise
