Requires nursing facilities participating in Medicaid to comply with a new direct care expense ratio mandating that at least 80% of patient care reimbursement be spent on direct care expenses such as laundry, housekeeping, dietary services, nursing administration, therapy services, and program services

Requires nursing facility license applications to disclose the names of all persons with 5% or more direct or indirect ownership interest in the facility, its parent/subsidiary companies, and the real property where the facility is located, with ongoing notification requirements for ownership changes

Authorizes the Health and Human Services Commission to recoup Medicaid reimbursement payments from nursing facilities that fail to meet the direct care expense ratio requirements

Establishes that providers subject to recoupment of incentive payments must be prohibited from participating in the nursing facility incentives program for at least two consecutive years, with a public list maintained on the commission's website

Exempts state-owned facilities from the direct care expense ratio requirements and prohibits making compliance with the ratio a condition of Medicaid participation
