Requires the Texas Department of Criminal Justice (TDCJ) to submit an annual report by January 31 to the governor, lieutenant governor, and legislature on the use of administrative segregation, including solitary confinement, death row confinement, and other restrictive housing

Report must include detailed data for each facility: offender demographics (name, race, sex, age), reasons for placement, frequency of placements, total time spent in segregation, and number of segregation beds used

Mandates reporting on mental health impacts, including diagnoses of segregated offenders, new mental health diagnoses or treatment needed during/after placement, instances of severe distress requiring medical/psychiatric attention, and suicide attempts or self-harm

Requires information on rehabilitative programs available to segregated offenders, staffing ratios in segregation units, placement appeals and outcomes, and reductions in segregation time due to program completion

TDCJ must submit a corrective action plan if reports are late or inaccurate; first report due January 31, 2027, with the act taking effect September 1, 2025
