Requires Texas birth certificates to include a space for recording biological sex as either male or female, determined by sex organs, chromosomes, or endogenous profile

Prohibits registrars and courts from issuing amended birth certificates that change the biological sex designation for minors, with limited exceptions

Allows exceptions for minors only to correct clerical errors, complete missing sex information, or update records for children born with atypical or ambiguous sex characteristics whose sex is later determined by a physician

Permits sex designation to be left blank at birth if a physician determines the child has atypical or ambiguous sex organs, chromosomes, or endogenous profile, with amendment allowed once sex is determined

Directs the Health and Human Services Commission to adopt implementing rules, with the act taking effect immediately upon two-thirds vote or September 1, 2025
