Authorizes counties with populations between 46,000 and 50,000 that are adjacent to Travis County (state capital) and not served by a hospital district or public hospital to establish a health care provider participation program

Allows commissioners courts to collect mandatory payments from nonpublic hospitals based on net patient revenue, with aggregate payments capped at 6% of total net patient revenue from all paying providers in the county

Requires collected funds to be deposited in a local provider participation fund and used primarily for intergovernmental transfers to provide the nonfederal share of Medicaid payments, including uncompensated care payments and rate enhancements for nonpublic hospitals

Limits county administrative expenses from the program to $20,000 annually plus collateralization costs, and prohibits hospitals from passing mandatory payments to patients as surcharges

Explicitly prohibits use of any funds resulting from the program to expand Medicaid eligibility under the Affordable Care Act
