Creates the Dementia Prevention and Research Institute of Texas to fund research on dementia and related disorders, develop prevention strategies, and attract research capabilities to Texas institutions

Establishes a 9-member Oversight Committee appointed by the governor (3), lieutenant governor (3), and speaker of the house (3), with members serving staggered 6-year terms and required to include physicians or scientists with dementia expertise

Caps grant awards at $300 million per state fiscal year, limits indirect costs to 5% of grant money, and requires grant recipients to provide matching funds equal to half the grant amount

Requires annual public reports and independent financial audits, with the institute subject to sunset review and abolishment on September 1, 2035, unless continued by the legislature

Takes effect December 1, 2025, contingent on voter approval of a constitutional amendment transferring $3 billion from state general revenue to the Dementia Prevention and Research Fund
