Establishes a loan program administered by the Health and Human Services Commission to help health care facilities purchase, implement, and sustain interoperable care coordination solutions, including electronic health record systems

Eligible facilities include nursing facilities, continuing care facilities, assisted living facilities, and mental health facilities; hospitals, abortion providers, diagnostic centers, boarding homes, and centers for independent living are excluded

Loans carry a maximum interest rate of 1% with terms up to 10 years; preferred facilities (those in medically underserved/rural areas or serving Medicaid recipients) may receive up to 80% of costs, while other facilities may receive up to 50%

Requires the executive commissioner to establish eligibility criteria, application procedures, loan guidelines, and compliance monitoring procedures through rulemaking

Allows the commission to accept gifts, grants, and donations from public and private entities to fund the program; effective date is September 1, 2025
