Requires the Texas Health and Human Services Commission to provide Medicaid reimbursement to providers offering nurse home visiting program services to pregnant women, postpartum women (up to 24 months after birth), and children 24 months of age or younger

Defines qualifying nurse home visiting programs as those that provide weekly or monthly in-home services through registered nurses, offer individualized health and education services, and meet federal Home Visiting Evidence of Effectiveness criteria administered by the U.S. Department of Health and Human Services

Directs the executive commissioner to establish a bundled Medicaid reimbursement rate covering education, screening, counseling, and resource referrals related to 19 specific service areas including healthy pregnancy, child development, postpartum depression, substance use, breastfeeding support, and domestic violence

Mandates that managed care organizations coordinate with nurse home visiting programs when a recipient loses Medicaid eligibility to arrange transfer of care plans and ensure continued coverage for program services

Takes effect September 1, 2025, with implementation contingent on obtaining any necessary federal waivers or authorizations
