Designates the act as the "Congresswoman Eddie Bernice Johnson Rehabilitation Hospital Care Ombudsman Act of 2025"

Expands the state long-term care ombudsman's authority to receive, investigate, and resolve complaints regarding rehabilitation hospital patients who may be in "imminent peril" (immediate danger of bodily injury, physical or mental impairment, or death) due to care provided or withheld by hospital staff

Grants the ombudsman office and representatives access to rehabilitation hospital patients and their medical records, with the same confidentiality protections that apply to long-term care facility residents

Requires rehabilitation hospitals to post signs in prominent locations and each patient room containing contact information for filing complaints with the ombudsman office, printed in at least 14-point sans serif typeface on 8.5x11 inch paper

Requires rehabilitation hospitals to cooperate with ombudsman investigations and mandates the Health and Human Services Commission to adopt implementing rules and publish a model complaint sign on its website
