Requires the Department of State Health Services to make the immunization exemption affidavit form available on its website and in all public schools, removing the previous requirement for a seal or security device to prevent reproduction

Changes department record-keeping from tracking affidavit forms "sent out" to tracking the number of times the form is accessed on the department's website each year

Maintains the prohibition on the department recording names of individuals who download or access the affidavit form

Repeals Section 161.0041(c) of the Health and Safety Code (the provision requiring individuals to request the form from the department)

Applies beginning with the 2025-2026 school year, with an effective date of September 1, 2025, or immediately upon receiving a two-thirds vote
