Requires the Health and Human Services Commission to provide Medicaid coverage to all individuals eligible for federal matching funds under the Affordable Care Act (ACA), effectively expanding Medicaid in Texas

Directs the executive commissioner to adopt rules implementing the expanded eligibility and to notify appropriate federal agencies of the expansion

Mandates an annual report to the governor, lieutenant governor, speaker of the house, and relevant legislative committees by December 1 each year on the effects of expansion

Annual report must address impacts on: number of uninsured Texans, state health care costs (including general revenue expenditures), local health care costs, and hospital charity/uncompensated care costs

Takes effect September 1, 2025, with provisions applying to eligibility determinations made on or after the implementation date
