Eliminates preauthorization requirements for breast reconstruction surgery, prostheses, and related treatments following mastectomy

Removes preauthorization requirements for inpatient care related to mastectomy procedures

Bars insurers from requiring preauthorization for diabetes equipment, supplies, and self-management training for qualified enrollees

Prohibits preauthorization requirements for bone mass measurements to detect osteoporosis risk, prostate cancer diagnostic examinations for males, and colorectal cancer screening examinations

Applies to health benefit plans delivered, issued, or renewed on or after January 1, 2026, with an effective date of September 1, 2025
