Extends the expiration date for health care provider participation programs to September 1, 2030, for qualifying counties, rather than the standard two-year expiration after program adoption

Applies specifically to counties with a population of one million or more that contain all or part of a municipality with one million or more residents

Further limits eligibility to counties located adjacent to a county with a population of 2.5 million or more

Maintains the existing two-year expiration rule for all other local governments operating health care provider participation programs

Takes effect immediately with a two-thirds vote of both legislative chambers, or September 1, 2025, otherwise
