Requires electronic health records in Texas to include a separate field for recording an individual's biological sex as either male or female, based on the sex observed and recorded by a health care provider at birth

Mandates that any algorithms or decision trees in electronic health records used to assist with medical treatment decisions must be based on the patient's recorded biological sex

Restricts amendments to the biological sex field to only two circumstances: correcting a clerical error, or when an individual is diagnosed with a sexual development disorder

Defines "biological sex" as the trait determining whether an organism produces male or female gametes, with "male" and "female" defined by reproductive system development for producing sperm or ova respectively

Requires the Health and Human Services Commission, Texas Medical Board, and Texas Department of Insurance to jointly implement and adopt rules for these requirements, applying to records prepared on or after the effective date
