Creates new Chapter 300B in the Health and Safety Code establishing general provisions applicable to health care provider participation programs operated by counties, municipalities, hospital districts, health care funding districts, and health care provider participation districts

Authorizes qualifying local governments to assess mandatory payments against institutional health care providers in their jurisdiction, with payments required to comply with federal regulations (42 C.F.R. Section 433.68(f)(3)(i)(A))

Caps mandatory payment revenue at either the amount authorized under the local government's specific program statute or an amount that, combined with all other hospital mandatory payments statewide, does not exceed 6% of aggregate net patient revenue from all Texas hospitals

Requires local governments using the higher revenue cap to notify the Health and Human Services Commission at least 20 days before setting payment amounts, with HHSC authorized to require reductions or procedural changes to ensure federal compliance

Prohibits mandatory payments from being construed as bed taxes or other state taxes, bars statewide hospital assessments, and allows HHSC to refuse intergovernmental transfers from non-compliant local governments
