Requires insurance entities offering Medicare supplement benefit plans to individuals 65 and older to also offer the same coverage to individuals under 65 who qualify for Medicare due to disability or end-stage renal disease

Mandates that premiums for under-65 Medicare enrollees must be the same rate charged to individuals who are 65 years old

Establishes enrollment periods for eligible individuals under 65: a 6-month period when first enrolling in Medicare Part B, a 60-day annual open enrollment starting on their birthday, and special enrollment periods designated by the commissioner

Prohibits insurers during enrollment periods from denying coverage, using medical underwriting, price discrimination based on health status, or imposing preexisting condition exclusions

Creates a transition enrollment period from September 1, 2025 through March 1, 2026 for individuals already enrolled in Medicare Part B due to disability or end-stage renal disease when the law takes effect
