Expands guardian application requirements to include phone number and date of birth of any person or institution having care and custody of the proposed ward or the proposed ward's estate, in addition to existing name and address requirements

Requires criminal background checks for persons who will have "care and custody" of the proposed ward rather than simply "contact with" the proposed ward, narrowing the scope of who must undergo checks

Exempts attorneys and certified guardians from direct criminal background check requirements, but mandates they provide identifying information for any non-certified person who will have care and custody of the ward on their behalf

Creates new ongoing notification requirement for guardians who are attorneys or certified guardians to inform the court within 30 days of any change in personnel having care and custody of the ward, including providing name, address, phone number, and date of birth of new designees

Requires court clerks to obtain criminal history record information from the Department of Public Safety and FBI for newly designated care and custody personnel when guardians report changes
