Creates Chapter 580 in the Health and Safety Code to regulate facilities that contract with the Health and Human Services Commission to provide inpatient competency restoration services for individuals ordered to undergo treatment to stand trial

Requires each contracted facility to enter into a memorandum of understanding with the county, municipality, local mental health authority, and local behavioral health authority where it is located to outline respective powers and duties

Mandates annual reporting from facilities to the Commission on metrics including: total individuals served, restoration success rates, average days of treatment, individuals restored within 60 days, treatment extensions sought, and transfers to mental health or residential care facilities

Requires the Commission to submit an annual report to the legislature by August 1 each year including facility performance evaluations, demographic data on individuals served, and cost comparisons between contracted facilities, state hospitals, and other competency restoration programs

Takes effect September 1, 2025, with the initial legislative report not required until August 1, 2027
