Requires the Texas Commissioner of Insurance to adopt rules establishing a uniform coordination of benefits questionnaire that all health benefit plan issuers in the state must use

Applies to a broad range of health benefit plans including individual and group insurance policies, HMOs, small employer plans, state employee plans, teacher plans, Medicaid managed care, CHIP, and self-funded plans sponsored by professional employer organizations

Mandates that health benefit plan issuers with coordination of benefits provisions use the standardized questionnaire and make it available to health care providers

Requires the Commissioner of Insurance to adopt the rules establishing the questionnaire by January 1, 2026

The uniform questionnaire requirement takes effect for use on or after February 1, 2026, with the overall act becoming effective September 1, 2025
