Requires the Health and Human Services Commission to issue licenses renewable every three years, with initial probationary licenses valid for one year, and mandates annual unannounced inspections of all licensed facilities

Imposes administrative penalties up to $10,000 per day per violation for noncompliance, civil penalties of $1,000 to $20,000 per violation threatening patient health and safety, and criminal penalties including fines up to $1,000 for operating without a license

Authorizes emergency license suspension or facility closure when violations create immediate threats to patient health and safety, and allows the commission to order suspension of admissions as a remedy

Sets implementation timeline requiring rule adoption by January 1, 2026, with full licensing requirements taking effect September 1, 2026
