Requires the Department of State Health Services to allow electronic submission of immunization exemption affidavit requests through its website, removing the previous written-only request requirement

Mandates the department to provide a standardized receipt confirming submission of an affidavit request, regardless of submission method (electronic, in person, mail, or fax)

Prohibits elementary, secondary schools, and child care facilities from denying admission to students who lack required immunizations if documentation shows an exemption affidavit request has been submitted to the department

Prohibits higher education institutions from denying admission to students lacking bacterial meningitis vaccinations or other required immunizations if documentation shows a pending exemption affidavit request

Takes effect immediately with two-thirds legislative vote, otherwise September 1, 2025
