Permanently adds all fentanyl-related substances to Schedule I of the Controlled Substances Act, covering any compound structurally related to fentanyl through specific chemical modifications

Creates expedited research registration procedures allowing practitioners with existing Schedule I or II registrations to begin research 30 days after notifying the Attorney General, and requiring action within 45 days for new applicants

Eliminates requirements for separate DEA registrations for researchers at the same institution and allows single registrations covering multiple research sites within the same city or county

Establishes enhanced criminal penalties for fentanyl-related substances by adding them to existing penalty provisions for fentanyl trafficking, covering quantities of 400 grams or more (10 years to life) and 40 grams or more (5-40 years)

Requires the Attorney General to issue implementing rules within 6 months of enactment, with authority to issue interim final rules that take effect immediately
