Extends the Acute Hospital Care at Home waiver program from January 30, 2026 to September 30, 2030, allowing Medicare patients to receive hospital-level acute care in their homes

Requires the Secretary of Health and Human Services to conduct a subsequent study by September 30, 2028 analyzing the hospital-at-home program's quality of care, costs, patient outcomes, readmission rates, and staffing compared to traditional inpatient settings

Mandates the study compare socioeconomic data of participants including race, ethnicity, income, housing, and dual Medicare-Medicaid eligibility status

Appropriates $2,500,000 to the Centers for Medicare & Medicaid Services for fiscal year 2026 to conduct the required study

Reduces the Medicare Improvement Fund from $1,403,000,000 to $1,400,500,000 to offset the study's cost
