Imposes a moratorium on new, renewed, or expanded federal authorizations for mountaintop removal coal mining in Kentucky, Tennessee, West Virginia, and Virginia until a federal health study determines no health risks exist for nearby communities

Directs the National Institute of Environmental Health Sciences, in consultation with EPA, to conduct a comprehensive study on health impacts of mountaintop removal mining on residents living near project sites

Requires mining operators to conduct continuous monitoring of water, air, and noise pollution, plus consistent soil pollution monitoring, and submit monthly results to the Secretary of Health and Human Services for public disclosure within 7 days

Authorizes the Office of Surface Mining Reclamation and Enforcement to assess a one-time fee on current and former mountaintop removal mining operators to cover federal implementation costs

Applies to permits issued under Section 404 of the Clean Water Act (Army Corps dredge/fill permits), Section 402 of the Clean Water Act (EPA discharge permits), and the Surface Mining Control and Reclamation Act of 1977
