Establishes a competitive grant program through HHS Administration for Children and Families to fund supportive services for residents of federally assisted affordable housing, with grants awarded for 5-year terms

Eligible recipients include entities with experience owning/managing affordable housing or delivering resident services, as well as tribes and tribal organizations, with priority given to mission-driven non-profit entities

Up to 75% of grant funds may cover service coordinator activities including health and mental health access, educational programs, financial literacy, aging-in-place support for seniors, independent living assistance for disabled residents, and benefits enrollment help

At least 25% of grant funds must be used for salary, benefits, and training for service coordinators

Applies to properties assisted under LIHTC, Section 8 project-based assistance, Section 202 elderly housing, Section 811 disability housing, HOPWA, rural housing programs, McKinney-Vento permanent supportive housing, and other federal affordable housing programs
