Prohibits U.S. foreign assistance funds from going to foreign governments that provided public funds for abortion, abortion promotion, abortion-related referrals, coerced abortions or sterilizations, or related biomedical research during the preceding fiscal year

Bars funding to foreign NGOs, international organizations, and multilateral entities (including WHO and UN Population Fund) that perform, promote, or provide referrals for abortions, regardless of funding source

Applies restrictions to U.S.-based organizations operating abroad and covers indirect funding through intermediaries, subgrantees, pooled funding mechanisms, and fungible budget support

Requires Secretary of State certification of recipient compliance before disbursing funds and annually thereafter, with mandatory suspension upon credible evidence of violations and investigations completed within 90 days

Violations result in termination of assistance, repayment of misused funds, minimum 3-year ineligibility for future assistance, and referral to Attorney General for potential civil or criminal prosecution; exceptions exist for abortions resulting from rape or incest with documentation
