HUD Secretary must establish a demonstration program within 180 days to award up to 10 grants to Continuum of Care programs for referring homeless individuals to Certified Community Behavioral Health Clinics for behavioral health, mental health, and substance use disorder treatment

Eligible grant recipients must be Continuum of Care programs located in one of the 5 states with the highest per capita homelessness rates and within 50 miles of a Certified Community Behavioral Health Clinic

Qualified participants include individuals receiving supportive housing through the Continuum of Care Program, those qualifying through Social Security disability or SSI benefits, or homeless individuals with disabilities under the McKinney-Vento Act

Authorizes $50 million for fiscal years 2025-2029, with up to 10% set aside for technical assistance and reporting

Requires HUD to submit a report to Congress within 180 days after program termination, including performance measures and data on participants receiving Social Security disability or SSI benefits
