Requires all health plans (group, individual, Medicare, Medicaid, TRICARE, and VA) to cover breast cancer screening and diagnostic imaging with no cost-sharing for individuals at increased risk of breast cancer or with dense breast tissue, effective January 1, 2026

Covered imaging includes 2D and 3D mammograms, breast ultrasounds, breast MRI, molecular breast imaging, and contrast-enhanced mammography at frequencies recommended by the National Comprehensive Cancer Network

Defines eligible individuals as those at increased breast cancer risk per American College of Radiology criteria or National Comprehensive Cancer Network guidelines, those with heterogeneously or extremely dense breast tissue, or those whose healthcare provider determines need additional screening based on age, race, ethnicity, or family history

Applies to grandfathered health plans under the Affordable Care Act and extends coverage requirements to ERISA-regulated employer plans and Internal Revenue Code provisions

Amends Medicare, Medicaid, Medicare Advantage, and military/veterans healthcare programs to eliminate copayments and cost-sharing for these breast cancer screenings for eligible individuals
