Authorizes $3 billion annually from the Highway Trust Fund for fiscal years 2027-2031 to fund the REPAIR Infrastructure Program, with $750 million designated for planning grants and $2.25 billion for capital construction grants

Renames and reauthorizes the Reconnecting Communities Pilot Program (established under the 2021 Infrastructure Investment and Jobs Act) as the REPAIR Infrastructure Program to address highways and transportation facilities that created barriers dividing communities

Adds new selection criteria for grant applicants including community participation plans, partnerships with community-based organizations, anti-displacement measures, affordable housing preservation, and creative placemaking initiatives

Prohibits grant funds from being used for projects that increase the number of travel lanes on existing highways

Makes REPAIR Infrastructure Program projects eligible for funding under multiple existing federal highway programs including the National Highway Performance Program, Surface Transportation Block Grant Program, Highway Safety Improvement Program, and Carbon Reduction Program
