Secretary of Veterans Affairs must establish a 3-year pilot program to coordinate care for veterans enrolled in both Medicare and VA health systems, administered through the VA's Center for Innovation for Care and Payment

Pilot program operates in 3-5 Veterans Integrated Service Networks with high veteran populations, including rural areas and medically underserved locations

Each participating veteran receives an assigned case manager responsible for individualized needs assessments, care coordination plans, and navigation between VA and Medicare systems

VA must contract with private sector entities experienced in commercial health care programs to assist with design, implementation, and care coordination to the greatest extent practicable

Secretary must track metrics including participant numbers, quality of care, costs, access standards, patient/provider satisfaction, and care coordination, with quarterly reports to Congress during the first two years
