Directs the CDC to establish a grant program within 1 year to fund PrEP (pre-exposure prophylaxis) programs for HIV prevention, with individual grants capped at $10,000,000 and $400,000,000 authorized annually for fiscal years 2027-2031

Eligible grant recipients include states, local governments, Indian Tribal governments, federally qualified health centers, rural health clinics, community-based organizations, and hospital or university-based clinics

Grant funds may cover PrEP medications, clinic and lab fees, office visits including telehealth, STD testing, counseling, case management, transportation, mental health services, and outreach activities

Requires HHS to establish a separate reimbursement program for uninsured individuals, including a "PrEP Pass" card to access HIV prevention services at no cost from registered providers

Grantees must contribute a 10% match (waivable for certain health centers and clinics), and Indian Tribal governments are exempt from matching requirements
