Elevates the Director of the Indian Health Service to a new position of Assistant Secretary for Indian Health within the Department of Health and Human Services, reporting directly to the HHS Secretary

Increases the number of HHS Assistant Secretaries from 6 to 7 and raises the position's pay level from Level V to Level IV on the federal Executive Schedule

Authorizes the new Assistant Secretary to appoint a Deputy Assistant Secretary and hire necessary officers, employees, and attorneys to administer Service activities

Provides that any existing person serving as IHS Director at the time of enactment shall continue serving as Assistant Secretary

Ensures all existing federal laws, executive orders, rules, and regulations referencing the IHS Director will automatically apply to the new Assistant Secretary position
